Quality happens only when someone is responsible for it.
Peter Principle: in a meritocratic company employees are promoted to their level of incompetence.
Package management resulting in bizzare webs of requirements like needing libTIFF to build Firefox even though Firefox does not render TIFF.
Peter Principle in Software: tools being made to solve problems that could have been standardized.
Quality only happens when someone (or a "dynamic duo") is responsible for it. Lack of quality happens when design committees are involved.
